Printing Industries of America members have provided the performance appraisal forms below for the exclusive use of other members. They should be used for informational purposes only and should NOT be considered as a standard practice or procedure. Prior to use, all performance appraisal forms should be customized to your company's procedures and positions.  Plus, have local legal counsel review for compliance with federal, state, and local laws.

We invite you to contribute your performance appraisal forms. Please remove any identifying references to your company or individuals. E-mail a MS Word, WordPerfect or ASCI file or copy and paste the text into the body of your e-mail to HRQuestions@printing.org. 

All performance appraisal forms become the property of Printing Industries of America, which reserves the right to edit or otherwise modify the listings. All references to specific companies or individuals will be removed. Only current members are allowed access to these sample performance appraisal forms.
